Dental Bill Format in India: What Every Dental Invoice Should Show

The dental clinic bill format used in India, field by field: what prevents billing disputes, and what should never appear on a patient's bill.

Most billing arguments in a dental clinic are not about the amount. They are about the patient not being able to see what they are paying for. A well-built invoice answers that before anyone asks.

Dental invoice checklist: the nine fields

  1. Clinic letterhead. The clinic's name and contact details, so the bill reads as an official document.
  2. Sequential invoice number. Generated automatically and never reused; it is what you and the patient refer to later.
  3. Invoice date. Separate from the treatment date if they differ.
  4. Patient name and record number (MRN). The MRN matters when two patients share a name.
  5. Each procedure, itemised. Procedure name, quantity and unit price on its own line.
  6. Treating doctor per line. Answers who did what, and underpins accurate dental doctor commission calculation.
  7. Any discount, shown openly. A visible discount is goodwill; one hidden inside a total is confusion.
  8. Total in INR. Subtotal, discount and final total in Rupees (₹).
  9. Paid and outstanding. The amount received, how it was paid, and the balance still due.

Check: ₹500 + ₹3,000 + ₹1,200 = ₹4,700; ₹4,700 − ₹3,000 = ₹1,700. The GST line is an example of stating the position explicitly; confirm the wording and document type with your accountant.

What should never appear on a dental bill

  • Internal procedure cost. What a treatment costs the clinic to deliver is a figure for margin and commission, not for the patient.
  • The lab bill as a separate charge. Lab cost normally sits inside the price quoted, not itemised as though the patient pays your supplier.
  • Doctor commission percentages. That is an arrangement between the clinic and the dentist.
  • Detailed clinical notes or diagnoses. An invoice is a financial document; findings belong in the dental clinical notes.

GST on dental invoices in India

What the law says. Notification No. 12/2017-Central Tax (Rate), Sr. No. 74 sets a Nil rate for health care services by a clinical establishment, an authorised medical practitioner or para-medics. Under section 31(3)(c) of the CGST Act, 2017, a registered person supplying exempted goods or services issues a bill of supply instead of a tax invoice (the linked consolidated text is updated to 30 September 2020; later amendments may apply).

What to check. Ordinary clinical dental treatment is therefore generally billed without GST. Cosmetic work that falls outside the notification's definition of health care, and goods sold separately, may be taxable. If one invoice mixes exempt and possibly taxable items, keep them as separate lines and confirm the treatment and document type with a chartered accountant or GST practitioner. Three billing habits worth adopting

  1. Invoice from the completed treatment, not from memory. If a finished procedure becomes an invoice line directly, the bill matches the clinical record.
  2. Keep outstanding balances visible. The front desk should know who owes money before the patient sits down.
  3. Use one print layout. Invoices and prescriptions on the same letterhead keep paperwork consistent.

There is no single dental bill template that every clinic must use, and the tax document required depends on where you practise. In India, if the clinic is registered for GST, the CGST Act sets which document it issues: section 31(3)(c) requires a bill of supply, instead of a tax invoice, for exempt supplies. Beyond that, the fields in this guide are a practical format that prevents disputes. Ask your chartered accountant what applies to your registration.

Not quite. An invoice states what is owed for the treatment; a receipt confirms money received. For staged treatment it is simplest when the invoice itself shows each payment and the balance, so it serves both purposes.

Each payment is recorded against the invoice as cash, card or bank transfer, and the balance updates. Dentrah records how the patient paid; it does not process UPI payments or connect to a payment gateway.
